CERTIFICATE MUST BE SIGNED. All signatures must be made by a duly-authorized officer, agent or other representative of broker or insurer, and shall certify the names of the insured, the additional insureds, the type and amount of insurance, the location and operations to which the insurance applies, and the expiration date of such insurance. A declaration page and list of endorsements and exclusions shall be included.
