Accidents. If a death, serious personal injury or substantial property damage occurs in connection with CONTRACTOR’s performance of this Agreement, CONTRACTOR shall immediately notify Mendocino County Risk Manager's Office by telephone. CONTRACTOR shall promptly submit to COUNTY a written report, in such form as may be required by COUNTY of all accidents which occur in connection with this Agreement. This report must include the following information: (1) name and address of the injured or deceased person(s); (2) name and address of CONTRACTOR's sub-contractor, if any; (3) name and address of CONTRACTOR's liability insurance carrier; and (4) a detailed description of the accident and whether any of COUNTY's equipment, tools, material, or staff were involved.
Accidents. Contractor shall file a written report with District in the event of any significant accident (i.e., any major accident regardless of whether any injury or damage occurs, an accident where any person is injured or dies, or any accident in which property is damaged or destroyed). Each accident report shall include all information relevant to the injury or damage, including, but not limited to:
(a) identification of the persons injured or the property damaged; (b) the extent or scope of the injury or damage; (c) all known facts regarding when, where and how the incident occurred; (d) identification of, and statements by, all persons who witnessed or were involved in the incident; (e) the weather and other conditions existing at the time and location of the incident; and (f) other information deemed relevant in connection with the incident. Contractor shall provide the accident report to District within twenty-four (24) hours of the incident. In addition, if the incident resulted in the serious injury of any person (something more than a minor cut, scratch or bruise) or substantial damage to any property (estimated to be in excess of Five Hundred Dollars ($500.00)), Contractor shall contact District immediately by telephone to report the incident.
A. Contractor shall at all times enforce strict discipline and good order among Contractor’s employees. Contractor and Contractor’s subcontractor(s) shall not employ any unfit person to perform any portion of the Work, or allow any unfit person to be present at, on or in the vicinity of the Site. A person shall be considered “unfit” if the person is not appropriately skilled for the tasks or work assigned to such person (other than designated apprentices working under the supervision of designated journeymen or other authorized supervisors); fails to comply with any rule or requirement in this Contract; creates any safety hazard or unreasonably fails to recognize any apparent safety hazard that might jeopardize that person, other persons or any property; or anyone who is unqualified or not competent to perform services under this Contract in District’s sole discretion.
B. Each person at, on or in the vicinity of the Site for any reason, including, but not limited to, any worker arriving for or departing from his or her work shift, shall at all times act in a courteous and professional manner. Accidents. The company will insure all employees covered by this Agreement against the loss of ordinary wages arising from work absence up to a period of 12 months due to injuries or illness resulting from any accident incurred in journeys between the employee’s residence and the workplace, and return. The company’s liability extends only to the reimbursement of the employee’s ordinary 38 hours rate and all such absences shall be supported by certification of a duly authorised medical practitioner.
Accidents. 26.3.1 All work covered by this agreement will be done in a factory, shop or place duly registered under the laws of the State but this will not prevent the employer bound by this agreement sending an employee from the factory, shop or place to any building or shop for the purposes of repairing, completing, fitting or fixing any work covered by this agreement.

Accidents. Any employee involved in any accident shall immediately report the accident and any physical injury sustained by any person involved. The employee shall make out an accident report on forms supplied by the Employer and shall tum in all available names and addresses of witnesses to the accident. Accident reports shall be presented in compliance with present policy, a copy of which shall be included in the Employer rules. Failure to comply with this provision shall subject an employee to disciplinary action by the Employer.
Accidents. Accidents for which the employee is at fault, or for which his action or lack of action is a contributory factor, will result in disciplinary action, which may range from "Reprimand" to "Dismissal" according to the seriousness of the accident, the degree of negligence or carelessness and frequency of accidents. Failure to report any accident as soon as possible will result in the employee's being subject to dismissal.
Accidents. All known job site and other project-related accidents, injuries, and illnesses sustained by A-E’s or subcontractors’ employees who require medical attention (other than first aid), shall be orally reported to COUNTY at the time of the incident. Written reports, satisfactory in form and content to COUNTY shall be submitted by A-E promptly after each such incident.
Accidents. In the event of any accident of any kind that involves the general public or property of the Village or a third party, the Contractor shall immediately notify the Director of Public Works by phone as well as provide Notice of the same. The Notice shall include a full accounting of all details of the accident. The Contractor shall furnish the Village with copies of all reports of such accidents at the same time that the reports are forwarded to any other interested parties.
Accidents. The contractor shall immediately on occurrence of any accident during carrying out the work report such accident to the Employer. The contractor shall also report such accident immediately to the concerned authorities whenever such report is required to be lodged by law and take appropriate actions thereof. The contractor shall submit report of all accidents, fires and property damage, dangerous occurrence to the authorized State Bank of India officials immediately after such occurrence, but in any case, not later than twelve hours of the occurrence. Such reports shall be furnished in the manner prescribed by the State Bank of India. In addition, the contractor to the authorized State Bank of India, official shall also submit periodic reports on safety from time to time as prescribed.
