Certification of Illness or Disability. The Corporation may require an employee to periodically provide medical certification at the employee’s expense during the employee’s illness, disability, or incapacity to work, or continuing illness, disability, or incapacity to work and the date when the employee is expected to be able to return to regular duties on a full or part-time basis. Such confirmation may be required in an acceptable form from the employee’s physician or the Medical Consultants of the Corporation. Failure to provide proper medical certification may result in the denial of Sick Leave or Long Term Disability benefits.
