Common use of Change in Entitlement Clause in Contracts

Change in Entitlement. Individual/small group instruction teachers whose family circumstances change entitling them only to single coverage must notify the Treasurer's office within thirty (30) days after the change occurs. The change in status will be effective the first of the following month. Failure to make timely notification shall result in the individual/small group instruction teacher being required to pay the difference in premium rates via payroll deduction.
