Change of Circumstances Reassessment during Period of Service. If during a Participant’s (excluding a Work Assist Participant's) Period of Service: the Participant’s individual circumstances change; or the Participant discloses information, such that the Program Services recommended in the Participant’s Current Assessment are no longer appropriate, the Provider must arrange for a Change of Circumstances Reassessment for the Participant to be conducted by DHS Assessment Services.
