Changes to Authorization. Covered Entity shall notify Business Associate of any changes in, or revocation of, permission by an individual to use or disclose Protected Health Information, to the extent that such changes may affect Business Associate's use or disclosure of Protected Health Information.
Changes to Authorization. Client shall notify Allscripts of any changes in, or revocation of, permission by an Individual to Use or Disclose PHI to the extent that such changes may affect Allscripts’ Use or Disclosure of PHI.
Changes to Authorization. If an authorization under Part III K 2 is no longer accurate because a different individual or position has responsibility for the overall operation of the construction activity, a new authorization satisfying the requirements of Part III K 2 shall be submitted to the VSMP authority as the administering entity for the board prior to or together with any reports or information to be signed by an authorized representative.
Changes to Authorization. If authorization under Part 7 B2 of this permit, is no longer valid due to a different individual or position being responsible for the overall operation of the facility; a new Signatory Authorization form, satisfying the requirements of Part 7, B 1, must be submitted to the city prior to, or with reports or applications to be signed by an authorized representative.
