Common use of CHECK OFF OF DUES, INITIATION FEES AND SERVICE FEES Clause in Contracts

CHECK OFF OF DUES, INITIATION FEES AND SERVICE FEES. The Employer shall deduct the Union Initiation Fee and Union Dues, and the Service Fee, when applicable, from the second paycheck of each calendar month for each employee from whom it receives written authorization to do so on the “Authorization for Payroll Deduction” form (Exhibit “A”) attached hereto and made part of this Agreement. The following terms and conditions shall apply to all such deductions:
