Chronic Care Management Program. For all eligible level 2 non-pregnant members the CONTRACTOR shall provide a Chronic Care Management Program. The goal of the program is to improve the quality of life, health status and utilization of services, of members with multiple chronic conditions, by providing intense self management education and support.
