Client Consent Form. A completed client consent form to participate in evaluation follow-up efforts must be collected and submitted prior to Service conclusion. Client refusal to participate in the follow-up survey must be documented in the client file.
Client Consent Form. A completed consent form to participate in admission survey, satisfaction survey and evaluation follow-up efforts must be administered and collected via the OHA PG Net data collection system. Refusal to participate in surveys must be documented in the client file.
Client Consent Form. Each client shall attest to voluntary participation in Program by providing written documentation of their informed consent. Contractor shall ensure that each client reads and understands the consent form, Program rules and supplies client with copies of the consent form and Program rules.
