Close case. A marked completion of the HCC program [as defined by the organization and approved by DSHS]. Upon completion of the HCC program, “close case” is selected in the CMBHS client profile screen. Clients who receive PSH must have a “close case” before the 30, and 90 day follow up that precedes discharge from service.
