Collection and Use of Information. In providing financial services to me, I understand you will be collecting and gathering personal, financial and credit information from me (Information) to: (i) to verify and/or authenticate my identity; (ii) better understand my financial situation and my needs and eligibility for products and services and manage my relationship with the Credit Union; (iii) open, maintain and administer my account and provide me with financial services that meet my needs and to conduct research and surveys to assess my satisfaction with the Credit Union, its products and services; (iv) obtain credit reports and evaluate my credit rating and credit worthiness and check references; (v) to administer and manage security and risk in relation to my account and the financial services provided to me; (vi) comply with legal and regulatory requirements; (vii) assist in dispute resolution; (viii) offer and provide me with the other products and services of the Credit Union and of its partners, affiliates and service suppliers. I understand that you require and may use my Social Insurance Number as an aid to identify me with credit bureaus and other financial institutions for credit matching purposes and for income tax reporting purposes on interest bearing or investment accounts. I understand that the provision of my Social Insurance Number for credit matching purposes is optional and not a condition of service. I understand that you need my consent to collect, use and disclose Information gathered about me except when the law allows you to do so without my consent. For that purpose, I authorize, consent to, and accept this as written notice of your obtaining, gathering, copying, scanning, updating, using, disclosing, sharing or exchanging such Information about me at any time for the purposes described including from or with any credit bureau, credit grantor or other entity in connection with my account and any relationships between us or those which you or I wish to establish and including with Credit Union partners, affiliates or service suppliers for the purposes described in (viii) above. You may use this Information for so long as it is needed for the purposes described. I understand that I can ask you to stop using my Information to offer me other products or services at any time. I also understand that I may request that you stop using my Social Insurance Number for credit matching purposes at any time. I understand it is necessary to keep my Information current and I agree to notify you of any changes in my Information. For the purpose of this authorization, your partners, affiliates and service suppliers mean Credit Union partners, affiliates and service suppliers that are engaged in the business of providing services or products to the public in Canada including but not limited to, deposits, financing arrangements, credit, charge and payment card service, trust and custodial services, securities and brokerage services, insurance services, electronic services, information and technology services, education and consulting services. To assist in providing financial services, the Credit Union may use cloud providers or other service providers located outside of Canada. In the event a cloud provider is used or a service provider is located outside of Canada, Information may be processed and stored outside of Canada and foreign governments, courts or law enforcement or regulatory agencies may be able to obtain disclosure of the Information through the laws of Canada and the foreign jurisdiction. If I am a US citizen, or I am a tax resident of the United States or another foreign jurisdiction, I understand the Credit Union may be required to disclose my Information to the Canada Revenue Agency (“CRA”) and CRA may share and exchange my Information with government, tax and other regulatory authorities of the foreign jurisdiction.

Collection and Use of Information. In providing financial services to me, I understand you will be collecting and gathering personal, financial and credit information from me (Information) to: (i) to verify meet regulatory and legal requirements; (ii) establish and/or authenticate my identity; (iiiii) better understand my financial situation protect me and the Credit Union from illegal activity; (iv) determine the suitability of products and services to me, and my needs and eligibility for products and services, including determining my eligibility for credit and receiving and exchanging Information on an ongoing basis with other credit suppliers and credit reporting agencies; (v) operate and administer products and services, which I have requested, including providing Information to related service providers involved in the operation and administration of those services and manage my relationship with on behalf of the Credit Union; (iiivi) open, maintain and administer my account and provide me with financial information or advice on products and services that meet my needs and may be of interest to conduct research and surveys to assess my satisfaction me (whether I currently have a product or service with the Credit Union, its ); (vii) conduct research to assist the Credit Union in designing products and services and determining products and services that may be of interest to me and to obtain my feedback on current products and services; (iv) obtain credit reports and evaluate my credit rating and credit worthiness and check references; (v) to administer and manage security and risk in relation to my account and the financial services provided to me; (vi) comply with legal and regulatory requirements; (vii) assist in dispute resolution; (viii) offer and provide me disclose Information to third parties in connection with the other products and services ongoing management of the Credit Union’s assets (including the assignment or sale of loans) and the further subsequent collection, use or disclosure of that Information by those third parties and any of their agents or assignees for the purposes of managing those assets; (ix) provide ombudsman or mediation services to address concerns with Credit Union and of its partners, affiliates and service suppliersproducts or services raised by me. I understand that you require and may use my Social Insurance Number as an aid to identify me with credit bureaus reporting agencies and other financial institutions for credit matching purposes and for income tax reporting purposes on interest bearing or investment accounts. I understand that the provision of my Social Insurance Number for credit matching purposes is optional and not a condition of service. I understand that you need my consent to collect, use and disclose Information gathered about me except when the law allows you to do so without my consent. For that purpose, I authorize, consent to, and accept this as written notice of your obtaining, gathering, copying, scanning, updating, using, disclosing, sharing or exchanging such Information about me at any time for the purposes described including from or with any credit bureaureporting agency, credit grantor or other entity in connection with my account and any relationships between us or those which you or I wish to establish and including with Credit Union partners, affiliates or service suppliers for the purposes described in (viii) aboveestablish. You may use this Information for so long as it is needed for the purposes described. I understand that I can ask you to stop using my Information to offer me other products or services at any time. I also understand that I may request that you stop using my Social Insurance Number for credit matching purposes at any time. I understand it is necessary to keep my Information current and I agree to notify you of any changes in my Information. For the purpose of this authorization, your partners, related service providers mean Credit Union affiliates and service suppliers mean with which the Credit Union partners, affiliates has entered into an agreement to provide products and service suppliers services to Credit Union members and customers under conditions that protect the privacy of Information and that are engaged in the business of providing services or products to the public in Canada including but not limited to, deposits, financing arrangements, credit, charge and payment card service, trust and custodial services, securities and brokerage services, insurance services, electronic services, information and technology services, education and consulting services. To assist in providing financial services, the Credit Union may use cloud providers or other service providers located outside of Canada. Collection and Use of Information. In From time to time, a) You may collect and use credit and other financially-related personal and business information ("Information") about me. This information may be received from me, from service arrangements I have made with or through you, from Credit Bureaus, credit reporting agencies, consumer or other reports, other financial institutions, and references I have provided to you. You may collect and use this information on an ongoing basis for the purposes of: identifying me, providing financial services, understanding my financial needs, protecting you from fraud and error, complying with legal and regulatory requirements and marketing products to me by any method of communication I have consented to; b) You may use and disclose the information as follows: i) You may give it to credit bureaus and other financial institutions and, with my consent, to other parties, ii) You may use it to determine my financial situation and credit history, iii) You may use it for any purpose related to the provision to me of financial services. You may also give it to anyone who works with or for you, but only as needed for the provision of those services, and iv) You may use my social insurance number for income tax reporting purposes if I have given that number to you; c) You may also use the Information for the following purposes: i) You may use it to promote your services to me. You may also add it to client lists you prepare and use for this purpose. ii) You may share it with Alterna Savings (where the law allows this) so that they may promote their services to me, I understand you will be collecting and gathering personal, financial and credit information from me (Information) to: (i) to verify and/or authenticate my identity; (ii) better understand my financial situation and my needs and eligibility for products and services and manage my relationship with the Credit Union; (and iii) open, maintain and administer my account and provide me with financial services that meet my needs and to conduct research and surveys to assess my satisfaction with the Credit Union, its products and services; (iv) obtain credit reports and evaluate my credit rating and credit worthiness and check references; (v) to administer and manage security and risk in relation to my account and the financial services provided to me; (vi) comply with legal and regulatory requirements; (vii) assist in dispute resolution; (viii) offer and provide me with the other products and services of the Credit Union and of its partners, affiliates and service suppliers. I understand that you require and You may also use my Social Insurance Number social insurance number as an aid to identify me with credit bureaus and other financial institutions for credit history file matching purposes and for income tax reporting purposes on interest bearing or investment accountspurposes. I understand that the provision of my Social Insurance Number for credit matching purposes is optional and not a condition of service. I understand that you need my consent to collect, use and disclose Information gathered about me except when the law allows may tell you to do so without my consentstop using the Information in ways described in subsection 17. For that purpose, I authorize, consent to, and accept this as written notice of your obtaining, gathering, copying, scanning, updating, using, disclosing, sharing or exchanging such Information about me (c) at any time for by contacting my branch or by calling the purposes Alterna Bank Contact Centre toll-free at 1.866.560.0120. You acknowledge that the use of Information in the ways described including from or with any in subsection 17. (c) is at my option and that I will not be refused credit bureau, credit grantor or other entity services just because I have told you to stop using it in connection with my account and any relationships between us those ways. I acknowledge, however, that you may require additional information in order to facilitate approval of credit or those which other services. If I am no longer your client or this Agreement terminates, you or I wish to establish and including with Credit Union partners, affiliates or service suppliers for the purposes described may keep information in (viii) above. You may use this Information for your records so long as it is needed for the purposes describeddescribed in subsection 16.(b) above. I understand that I can ask you to stop using my Information to offer me other products or services at any time. I also understand that I may request that you stop using my Social Insurance Number for credit matching purposes at any time. I understand it Alterna Bank is necessary to keep my Information current and I agree to notify you of any changes in my Information. For the purpose of this authorization, your partners, affiliates and service suppliers mean Credit Union partners, affiliates and service suppliers that are engaged in the business of providing services or products an entity subject to the public in Canada including but not limited toProceeds of Crime (Money Laundering) and Terrorist Financing Act and is required by law to collect the name, depositsaddress, financing arrangements, credit, charge date of birth and payment card service, trust and custodial services, securities and brokerage services, insurance services, electronic services, information and technology services, education and consulting services. To assist in providing financial services, the Credit Union may use cloud providers or other service providers located outside of Canada. In the event a cloud provider is used or a service provider is located outside of Canada, Information may be processed and stored outside of Canada and foreign governments, courts or law enforcement or regulatory agencies may be able to obtain disclosure occupation of the Information through account holder along with the laws of Canada and the foreign jurisdiction. If I am a US citizen, or I am a tax resident of the United States or another foreign jurisdiction, I understand the Credit Union may be required foundation documentary evidence to disclose my Information to the Canada Revenue Agency (“CRA”) and CRA may share and exchange my Information with government, tax and other regulatory authorities of the foreign jurisdictionprove identity.

In providing financial services to me, I understand you The Borrower understands and acknowledges that the Credit Union will be collecting and gathering personal, financial and credit information from me and about the Borrower (Information) to: : (i) to verify and/or authenticate my the Borrower's identity; ; (ii) obtain credit reports and evaluate the Borrower’s credit rating and credit worthiness and check references; (iii) better understand my the Borrower’s financial situation and my make decisions about loan or credit applications; (iv) administer, monitor or service the Borrower’s account and collect the Borrower’s loan; (v) administer and manage security and risk in relation to the Borrower’s loan and any of the Borrower’s accounts and the financial services provided to the Borrower; (vi) comply with legal, security and regulatory requirements; (vii) assist in dispute resolution; (viii) better understand the Borrower’s needs and eligibility for products and or services and manage my the Borrower's relationship with the Credit Union; ; (iiiix) open, maintain and administer my account and provide me with financial services that meet my needs and to conduct research and surveys to assess my the Borrower's satisfaction with the Credit Union, its products and services; ; (iv) obtain credit reports and evaluate my credit rating and credit worthiness and check references; (v) to administer and manage security and risk in relation to my account and the financial services provided to me; (vi) comply with legal and regulatory requirements; (vii) assist in dispute resolution; (viiix) offer and provide me the Borrower with the other products and services of the Credit Union and of its partners, affiliates and service suppliers. I understand that you require The Borrower understands the Credit Union requires and may use my the Borrower’s Social Insurance or Business Number as may be applicable as an aid to identify me the Borrower with credit bureaus and or other financial institutions for credit matching purposes and or for income tax reporting purposes on interest bearing or investment accountspurposes. I understand The Borrower also understands that the provision of my the Borrower’s Social Insurance Number for credit matching purposes is optional and is not a condition of service. I understand The Borrower understands that you need my the Credit Union needs the Borrower's consent to collect, use and disclose Information gathered about me the Borrower except when the law allows you the Credit Union to do so without my the Borrower’s consent. For that purpose, I authorizethe Borrower authorizes, consent consents to, and accept accepts this as written notice of your the Credit Union obtaining, gathering, copying, scanning, updating, using, disclosing, sharing or exchanging such Information about me the Borrower at any time for the purposes described including from or with any credit bureau, government agency, credit grantor or other entity in possession of such Information and the Borrower specifically directs and authorizes such entities to provide Information at the Credit Union’s request. The Credit Union is also authorized to continually update, obtain and use Information at any time in connection with my account and any relationships between us or those which you or I wish to establish and the Borrower’s loan, including with enforcement purposes. The Credit Union partners, affiliates may share and exchange Information with any guarantor of the loan or service suppliers for any other lender or credit grantor that is participating in the purposes described in (viii) aboveloan or who may receive an assignment of all or part of the loan. You The Credit Union may use this Information for so long as it is needed for the purposes described. I understand The Borrower understands that I the Borrower can ask you the Credit Union to stop using my the Information to offer me other products or services at any time. I The Borrower also understand understands that I the Borrower may request that you ask the Credit Union to stop using my the Borrower’s Social Insurance Number for credit matching purposes at any time. I understand The Borrower understands it is necessary to keep my the Borrower’s Information current and I agree the Borrower agrees to notify you the Credit Union of any changes in my the Borrower’s Information. For the purpose of this authorization, your Credit Union partners, affiliates and service suppliers mean Credit Union partners, affiliates and service suppliers that are engaged in the business of providing services or products to the public in Canada including including, but not limited to, deposits, financing arrangements, credit, charge and payment card serviceservices, trust and custodial services, securities and brokerage services, insurance services, electronic services, information and technology services, education educational and consulting services. For the purpose of this authorization, your partners, affiliates and service suppliers mean Credit Union partners, affiliates and service suppliers that are engaged in the business of providing services or products to the public in Canada including but not limited to, deposits, financing arrangements, credit, charge and payment card service, trust and custodial services, securities and brokerage services, insurance services, electronic services, information and technology services, education and consulting services. Collection and Use of Information. In From time to time, a. You may collect and use credit and other financially related personal and business information (“Information”) about me. This Information may be received from me from service arrangements I have made with or through you, and from credit bureaus, credit reporting agencies, consumer or other reports, other financial institutions, and references that I provided to you. You may collect and use this Information on an ongoing basis for the purposes of: identifying me, providing financial services to meservices, I understand you will be collecting and gathering personal, financial and credit information from me (Information) to: (i) to verify and/or authenticate my identity; (ii) better understand understanding my financial situation and my needs and eligibility for products situation, protecting us from fraud and services and manage my relationship with the Credit Union; (iii) openerror, maintain and administer my account and provide me with financial services that meet my needs and to conduct research and surveys to assess my satisfaction with the Credit Union, its products and services; (iv) obtain credit reports and evaluate my credit rating and credit worthiness and check references; (v) to administer and manage security and risk in relation to my account and the financial services provided to me; (vi) comply complying with legal and regulatory requirements; (vii) assist , and marketing products to me by any method of communication I have consented to. b. You may use and disclose the information as follows: i. You may give it to credit bureaus and other financial institutions and, with my consent, to other parties. ii. You may use it to determine my financial situation and credit history. iii. You may use it for any purpose related to the provision to me of financial services. iv. You may also give it to anyone who works with or for you or to any third party service providers in dispute resolution; (viii) offer and or outside of Canada that work with you to provide me with financial services (such as, card and Instrument issuers, information technology service providers, marketing service providers, information storing or processing service providers, etc.), but only as needed for the other products and services provision of the Credit Union and of its partners, affiliates and service suppliers. I understand that you require and those services. v. You may use my Social Insurance any personal identifier number, such as a Master Business Licence Number, CRA Business Number, Registered Charity Number, or any number of a similar nature for income tax reporting purposes if I have given that number to you. vi. You may use the Information to conduct investigations of possible breaches of law; to identify, contact, or bring legal action against someone who may be violating an agreement they have with you; to investigate security breaches or cooperate with government authorities pursuant to a legal matter; or to protect your rights, safety or property. vii. You may use the Information for other purposes which are permitted or required by law, or which are disclosed to me and to which I consent. c. You may also use the Information for the following purposes: i. You may use it to promote your services to me. You may also add it to client lists you prepare and use for this purpose. ii. You may share it with Alterna Bank (where the law allows this) so that they may promote their services to me. iii. You may also use any personal identifier number, such as a Master Business Licence Number, CRA Business Number, Registered Charity Number, or SIN Number if I have given that number to you or any number of a similar nature as an aid to identify me with credit bureaus and other financial institutions for credit history file matching purposes and for income tax reporting purposes on interest bearing or investment accountspurposes. I understand that the provision of my Social Insurance Number for credit matching purposes is optional and not a condition of service. I understand that you need my consent to collect, use and disclose Information gathered about me except when the law allows may tell you to do so without my consent. For that purpose, I authorize, consent to, and accept this as written notice stop using the Information in ways described in subsection 29.c of your obtaining, gathering, copying, scanning, updating, using, disclosing, sharing or exchanging such Information about me the Agreement at any time for by contacting my branch or by contacting your Contact Centre at the purposes telephone number located at the top of this page. You acknowledge that the use of Information in the ways described including from or with any in subsection 29.c of this Agreement is at my option and that I will not be refused credit bureau, credit grantor or other entity services just because I have told you to stop using it in connection with my account and any relationships between us those ways. I acknowledge, however, that you may require additional information in order to facilitate approval of credit or those which other services. If I am no longer your client or this Agreement terminates, you or I wish to establish and including with Credit Union partners, affiliates or service suppliers for the purposes described may keep information in (viii) above. You may use this Information for your records so long as it is needed for the purposes described. I understand that I can ask you to stop using my Information to offer me other products or services at any time. I also understand that I may request that you stop using my Social Insurance Number for credit matching purposes at any time. I understand it is necessary to keep my Information current and I agree to notify you of any changes described in my Information. For the purpose subsection 29.b of this authorization, your partners, affiliates and service suppliers mean Credit Union partners, affiliates and service suppliers that are engaged in the business of providing services or products Agreement. Alterna Savings is an entity subject to the public in Canada including but not limited toProceeds of Crime (Money Laundering) and Terrorist Financing Act and is required by law to collect the name, depositsaddress, financing arrangements, credit, charge date of birth and payment card service, trust and custodial services, securities and brokerage services, insurance services, electronic services, information and technology services, education and consulting services. To assist in providing financial services, the Credit Union may use cloud providers or other service providers located outside of Canada. In the event a cloud provider is used or a service provider is located outside of Canada, Information may be processed and stored outside of Canada and foreign governments, courts or law enforcement or regulatory agencies may be able to obtain disclosure occupation of the Information through account holder along with the laws of Canada and the foreign jurisdiction. I understand that you require and may use my Social Insurance Number as an aid to identify me with credit bureaus reporting agencies and other financial institutions for credit matching purposes and for income tax reporting purposes on interest bearing or investment accounts.
