Concerns and/or requests should be addressed between the Contractor and Building Manager or Director of Facilities Management. A resolution shall be addressed by the contractor within 24 hours. If there is a performance deficiency, a Corrective Action Report (CAR) may be used. Complete this form to report concerns with vendors or commodities.
