This form is located on State website at the following location: xxxxx://xxx.xxxxxxxx.xxx/media/7349/coi-guidance-doc-lpa.pdf Consultants and subconsultants providing services for LPA’s, or submitting proposals for services, shall submit a Conflict of Interest Disclosure Form for Consultants. Consultants and subconsultants shall submit a revised form for any changes in circumstances, or discovery of any additional facts that could result in someone employed by, or who has an ownership, personal, or other interest with Consultant or subconsultant having a real or potential conflict of interest on an LPA federal-aid transportation project.
