COORDINATE WITH OTHER AGENCIES. A. In partnership with other members of the Mendocino County Continuum of Care system for the homeless, develop and provide, at minimum: 1. An individualized program of life skills trainings and support for each household or individual living in transitional housing: a. Substance abuse education b. Parenting skills training c. Employment training 2. Requirements that each individual and family household will be required to save money for move-in costs to permanent housing B. Work with the SHP/STEP program and the SHP Case Conference Team to provide on-going assessments and updated service plans for residents of transitional housing C. Participate with SHP/Shelter Plus Care Management Team in the review of existing and development of new policies and procedures for all services and supportive housing within the STEP Continuum of Care
