Description of Benefits. Subject to the exclusions, conditions, and limitations of this Agreement, and subject to Member is entitled to the benefits described in this Section for Medically Necessary and Appropriate Services rendered by a Provider and/or Supplier in the amounts specified in SECTION SB - SCHEDULE OF BENEFITS of this Agreement. A different level of benefits may be provided based on the place of treatment as set forth in SECTION SB - SCHEDULE OF BENEFITS of this Agreement.
