DESCRIPTION OF SERVICES, STAFFINfi. 5 4.1 CONTRACTOR agrees to provide those services, facilities, equipment 6 and supplies as described in the Exhibit “A” to the Agreement Between County 7 of Orange El Modena Family Resource Center, for the Provision of Services 8 Promoting Safe and Stable Families Services, attached hereto and incorporated 9 herein by reference. CONTRACTOR shall operate continuously throughout the 10 term of this Agreement with the number and type of staff described and as 11 required for provision of services hereunder pursuant to the personnel 12 disclosure provisions of this Agreement. 13 4.2 Subject to thirty (30) days written notice, ADMINISTRATOR may, in 14 his or her sole discretion, require changes in staffing allocations to reflect 15 current workload demands or service needs as long as COUNTY’s maximum 16 obligation as set forth in this Agreement is not exceeded. 17 4.3 Upon the request of ADMINISTRATOR, CONTRACTOR shall send 18 appropriate staff to attend an orientation session and subsequent training 19 sessions given by COUNTY.

CONTRACTOR shall operate continuously throughout the 10 term of this Agreement with the number and type of staff described and as 11 required for provision of services hereunder pursuant to the personnel 12 disclosure provisions of this Agreement. 13 4.2 Subject to thirty (30) days written advance notice, ADMINISTRATOR may, in 14 his or her sole discretion, require changes in staffing allocations to reflect 15 current workload demands or service needs as long as COUNTY’s maximum 16 obligation as set forth in this Agreement is not exceeded. 17 4.3 Upon the request of ADMINISTRATOR, CONTRACTOR shall send appropriate 18 appropriate staff to attend an orientation session and subsequent training 19 sessions given 19 by COUNTY.

DESCRIPTION OF SERVICES, STAFFINfi. 5 4.1 25 5.1 CONTRACTOR agrees to provide those services, facilities, equipment 6 26 and supplies as described in the Exhibit “A” to the Agreement Between County 7 27 of Orange El Modena Family Resource Centerand A White and Yellow Cab Inc., for the Provision of Services 8 Promoting Safe and Stable Families Children’s 28 School of Origin Transportation Services, attached hereto and incorporated 9 1 herein by reference. CONTRACTOR shall operate continuously throughout the 10 2 term of this Agreement with the number and type of staff described and as 11 3 required for provision of services hereunder pursuant to the personnel 12 4 disclosure provisions of this Agreement. 13 4.2 5 5.2 Subject to thirty (30) days written advance notice, ADMINISTRATOR may, in 14 6 his or her sole discretion, require changes in staffing allocations to reflect 15 7 current workload demands or service needs as long as COUNTY’s maximum 16 8 obligation as set forth in this Agreement is not exceeded. 17 4.3 9 5.3 Upon the request of ADMINISTRATOR, CONTRACTOR shall send 18 10 appropriate staff to attend an orientation session and subsequent training 19 11 sessions given by COUNTY.

CONTRACTOR shall operate continuously 9 throughout the 10 term of this Agreement with the number and type of staff 10 described and as 11 required for provision of services hereunder pursuant to the 11 personnel 12 disclosure provisions of this Agreement. 13 12 4.2 Subject to thirty (30) days advance written notice, ADMINISTRATOR 13 may, in 14 his or her sole discretion, require changes in staffing allocations to 14 reflect 15 current workload demands or service needs as long as COUNTY’s maximum 16 15 obligation as set forth in this Agreement is not exceeded. 17 16 4.3 Upon the request of ADMINISTRATOR, CONTRACTOR shall send 18 17 appropriate staff to attend an orientation session and subsequent training 19 18 sessions given by COUNTY.

CONTRACTOR shall operate continuously 10 throughout the 10 term of this Agreement with the number and type of staff 11 described and as 11 required for provision of services hereunder pursuant to the 12 personnel 12 disclosure provisions of this Agreement. 13 4.2 Subject to thirty (30) days written advance notice, ADMINISTRATOR may, in 14 his or her sole discretion, require changes in staffing allocations to reflect 15 current workload demands or service needs as long as COUNTY’s maximum 16 obligation as set forth in this Agreement is not exceeded. 17 4.3 Upon the request of ADMINISTRATOR, CONTRACTOR shall send 18 appropriate staff to attend an orientation session and subsequent training 19 sessions given by COUNTY.

DESCRIPTION OF SERVICES, STAFFINfi. 5 4.1 CONTRACTOR agrees to provide those services, facilities, equipment 6 and supplies supplies, as described in the Exhibit “A” to the Agreement Between between County 7 of Orange El Modena Family Resource Center, for the Provision of Services 8 Promoting Safe and Stable Families Care Services, attached hereto and incorporated 9 herein by reference.: Exhibit “A” relating to the Scope of Work, and Exhibit “B” relating to the Definitions. CONTRACTOR shall operate continuously throughout the 10 term of 7 of Orange and New Alternatives, Incorporated, for the Provision of Respite 11 this Agreement with the number and type of staff described and as 11 required for 12 provision of services hereunder pursuant to the personnel 12 disclosure provisions of this Agreementhereunder. 13 4.2 Subject to thirty (30) days advance written notice, ADMINISTRATOR may, in 14 his or her sole discretion, may require changes in staffing allocations to reflect 15 current workload 15 demands or service needs as long as COUNTY’s maximum 16 obligation as set forth 16 in this Agreement is not exceeded. 17 4.3 Upon the request of ADMINISTRATOR, CONTRACTOR shall send 18 appropriate staff to attend an orientation session and subsequent training 19 sessions given by COUNTY.

DESCRIPTION OF SERVICES, STAFFINfi. 5 4.1 11 5.1 CONTRACTOR agrees to provide those services, facilities, equipment 6 12 and supplies as described in the Exhibit “A” to the Agreement Between between County 7 of 13 Orange El Modena Family Resource Centerand New Alternatives, Inc., for the Provision of Transitional 14 Residential Home Services 8 Promoting Safe and Stable Families Servicesat Tustin Family Campus, attached hereto and 15 incorporated 9 herein by reference. CONTRACTOR shall operate continuously 16 throughout the 10 term of this Agreement with the number and type of staff 17 described and as 11 required for provision of services hereunder pursuant to the 18 personnel 12 disclosure provisions of this Agreement. 13 4.2 19 5.2 Subject to thirty (30) days written notice, ADMINISTRATOR may, in 14 20 his or her sole discretion, require changes in staffing allocations to reflect 15 21 current workload demands or service needs as long as COUNTY’s maximum 16 22 obligation as set forth in this Agreement is not exceeded. 17 4.3 23 5.3 Upon the request of ADMINISTRATOR, CONTRACTOR shall send 18 24 appropriate staff to attend an orientation session and subsequent training 19 25 sessions given by COUNTY.

DESCRIPTION OF SERVICES, STAFFINfi. 5 4.1 CONTRACTOR agrees to provide those services, facilities, equipment 6 equipment, and supplies as described in the Exhibit “A” to the Agreement Between 7 between County 7 of Orange El Modena Family Resource Centerand Medtox Laboratories, Inc. for the Provision of Services 8 Promoting Safe and Stable Families Drug Testing Services, attached hereto and incorporated 9 herein by reference. 9 CONTRACTOR shall operate continuously throughout the 10 term of this Agreement 10 with the number and type of staff described and as 11 required for provision of 11 services hereunder pursuant to the personnel 12 disclosure provisions of this 12 Agreement. 13 4.2 Subject to thirty (30) days written advance notice, ADMINISTRATOR may, in 14 his or her sole discretion, require changes in staffing allocations to reflect 15 current workload demands or service needs as long as COUNTY’s maximum 16 obligation as set forth in this Agreement is not exceeded. 17 4.3 Upon the written request of ADMINISTRATOR, CONTRACTOR shall send 18 appropriate staff to attend an orientation session and subsequent training 19 sessions given by COUNTY, not to exceed 19 one training session per year and solely related to the services provided by 20 CONTRACTOR.

DESCRIPTION OF SERVICES, STAFFINfi. 5 4.1 CONTRACTOR agrees to provide those services, facilities, 6 equipment 6 and supplies as described in the Exhibit “A” to the Agreement 7 Between County 7 of Orange El Modena and La Habra Family Resource Center, for the 8 Provision of Services 8 Promoting Safe and Stable Families Services, attached 9 hereto and incorporated 9 herein by reference. CONTRACTOR shall operate 10 continuously throughout the 10 term of this Agreement with the number and type of 11 staff described and as 11 required for provision of services hereunder pursuant 12 to the personnel 12 disclosure provisions of this Agreement. 13 4.2 Subject to thirty (30) days written notice, ADMINISTRATOR may, 14 in 14 his or her sole discretion, require changes in staffing allocations to 15 reflect 15 current workload demands or service needs as long as COUNTY’s maximum 16 obligation as set forth in this Agreement is not exceeded. 17 4.3 Upon the request of ADMINISTRATOR, CONTRACTOR shall send 18 appropriate staff to attend an orientation session and subsequent training 19 sessions given by COUNTY.
