Common use of Diabetes Management Program Clause in Contracts

Plan participants diagnosed with diabetes, who participate in one of the plans’ Diabetes Disease Management programs will have their co-payments waived on diabetes medications and test strips for the following calendar year as long as they maintain their participation in the diabetes management program and use their medications prescribed to treat diabetes without missing any days.
