Disbursement of Financial Assistance. Unless a different disbursement method is (a) Under this FAA, Financial Assistance shall be drawn in the order specified in Section 3.01(d) of this document.
(b) Each disbursement request shall be delivered to DNR. Each request must contain invoices or other evidence acceptable to DNR and DOA that line Project Costs for which disbursement is requested have been incurred by the Commission.
(c) The CWFP, through its agents or Trustee, plans to make disbursements of Exhibit C, “Financial Assistance Award,” OHA will on a semimonthly basis upon approval of each disbursement request by DNR and DOA. Such approval by DNR and DOA may require adjustment and corrections to the disbursement request submitted by the Commission. DNR or DOA shall notify the Commission whenever such an adjustment or correction is made by DNR or DOA.
(d) Disbursements made to the Commission are subject to pre- and post-payment adjustments by DNR or DOA.
(1) If the Financial Assistance is not yet fully disbursed, and CWFP funds were previously disbursed for costs not eligible for CWFP funding or not eligible under this FAA, the CWFP shall make necessary adjustments to future disbursements.
(2) If the Financial Assistance is fully disbursed, including disbursements for any costs not eligible for CWFP funding or not eligible under this FAA, the Municipality, as a member of the Commission, agrees to repay to the CWFP an amount equal to the non-eligible costs within 60 days of notification by DNR or DOA. The CWFP shall then apply the amount it receives as a Loan prepayment or as a recovery of a Loan disbursement with Principal Forgiveness (if there is no outstanding Loan principal balance available to which the recovery may be applied).
(e) The CWFP or its agent shall disburse Financial Assistance only to the Part A awards for MHS 28 Services provided under a particular line Commission’s account by electronic transfer of funds. The Municipality hereby covenants that it shall take actions and provide information necessary to facilitate these transfers.
(f) Disbursement beyond ninety-five percent (95%) of the Financial Assistance Award containing an “A” in column “Part ABC” Assistance, unless otherwise agreed to County in substantially equal monthly allotments during by DNR and DOA pursuant to a written request from the period specified in that line of Municipality or the Financial Assistance AwardCommission, subject to the followingmay be withheld until:
i. OHA may, after 30 days (unless parties agree otherwise1) written notice to County, reduce DNR is satisfied that the monthly allotments based on under- used allotments identified through MOTS and other reports Project has been completed in accordance with the “Reporting Requirements” Plans and “Special Reporting Requirements” sections above or applicable special conditionsSpecifications, DNR has approved all change orders relating to the Project, and DNR has determined that the Project is in compliance with the Commission’s WPDES Permit;
ii. OHA may, upon written request (2) the Commission certifies to DNR its acceptance of County, adjust monthly allotmentsthe Project from its contractors;
iii. Upon amendment (3) the Commission certifies in writing to the Financial Assistance Award, OHA shall adjust monthly allotments DNR its compliance with applicable federal requirements (certification must be as necessary, to reflect changes in the funds awarded for MHS 28 Services provided under that line of the Financial Assistance Award;
iv. OHA is not obligated to provide financial assistance for any MHS 28 Services that are not properly reported in accordance with the “Reporting Requirements” and “Special Reporting Requirements” sections above or as required in an applicable Specialized Service Requirement by the date 60 calendar days after the earlier of expiration or termination of this Agreement; termination of OHA’s obligation to provide financial assistance for MHS 28 Services; or termination of County’s obligation to include the Program Area in which MHS 28 Services fall within its CMHPprescribed on Exhibit G); and
v. OHA will reduce (4) DNR certifies in writing to DOA the financial assistance awarded for MHS 28 Services delivered under a particular line Municipality’s and the Commission’s compliance with all applicable requirements of Exhibit C, “Financial Assistance Award,” containing an “A” in column “Part ABC,” by the amount received by a Provider of MHS 28 Services as payment of a portion of the cost of the Services from an Individual receiving such Services with funds awarded in that line of the Financial Assistance Awardthis FAA.

OHA will reduce the financial assistance awarded for MHS 28 Services delivered under a particular line of Exhibit C, “Financial Assistance Award,” containing an “A” in column “Part ABC,” by ”, the parties will amend the amount received by a Provider of MHS 28 Services as payment of a portion of the cost of the Services from an Individual receiving such Services with funds awarded for A&D 82 Services in that line in proportion to the underutilization during that period, including, but not limited to, reducing the amount of future funds awarded for A&D 82 Services in an amount equal to funds reduced under that line due to underutilization. For purposes of documenting the revised amount of services, County and OHA shall execute an appropriate amendment to the Financial Assistance AwardAward to reflect this reduction.
(5) County may, with OHA approval, apply A&D 82 allotments for Services not provided in the first fiscal year toward A&D 82 Services in the second fiscal year.

(e) OHA will reduce the financial assistance awarded provided for MHS 28 A&D 61 Services delivered under a particular line of Exhibit C, “Financial Assistance Award,” containing an “A” in column “Part ABC,” by the amount received by a Provider of MHS 28 Services A&D 61 Services, as payment of a portion of for the cost of the Services delivered to an Individual, from an the Individual, the Individual’s health insurance provider, another person’s health insurance provider under which Individual receiving such is also covered, or any other Third Party Resource (TPR) in support of Individual’s care and Services with funds awarded in that line provided. County is obligated to report to OHA, by email at xxxxxxxxxxx.xxxxxxxxxxxxx@xxxxxx.xxxxx.xx.xx, any TPR payments no later than 30 calendar days following receipt of the Financial Assistance Awardpayment by County or Service Provider. OHA is not obligated to provide financial assistance for any MHS 28 Services that are not properly reported in accordance with the “Reporting Requirements” and “Special Reporting Requirements” sections above or as required in an applicable Specialized Service Requirement by the date 60 calendar days after the earlier of expiration or termination of this Agreement; termination of (d) OHA’s obligation to provide financial assistance under this Agreement is subject to the satisfaction of the County delivering the anticipated level of Service upon which the allotments were calculated. If for MHS 28 Services; or termination a period of County’s obligation to include three consecutive months, during the Program Area term of this Agreement, County delivers less than the anticipated level of Service upon which allotments were calculated in which MHS 28 Services fall within its CMHP; and
v. OHA will reduce the financial assistance awarded for MHS 28 Services delivered under a particular line of Exhibit C, “Financial Assistance Award,” containing an “A” in column “Part ABC,” by the parties may amend the amount received by a Provider of MHS 28 Services as payment of a portion of the cost of the Services from an Individual receiving such Services with funds awarded for A&D 82 Services in that line in proportion to the underutilization during that period including, but not limited to, reducing the amount of future funds awarded for A&D 82 Services in an amount equal to funds reduced under that line due to underutilization. For purposes of documenting the revised amount of services, County and OHA shall execute an appropriate amendment to the Financial Assistance AwardAward to reflect this reduction; and
(e) County may, with OHA approval, apply allotments for A&D 82 Services not provided in the first fiscal year toward A&D 82 Services in the second fiscal year.

Disbursement of Financial Assistance. Unless a different disbursement method is specified in that line of Exhibit C, “Financial Assistance Award,” OHA will disburse the Part A awards funds awarded for MHS 28 05 Services provided under a particular line of the Exhibit C, “Financial Assistance Award Award,” containing an “A” in column “Part ABC,” to County in substantially equal monthly allotments during the period specified in that line of the Financial Assistance Award, subject to the following:
i. (a) OHA may, upon written request of County, adjust monthly allotments;
(b) Upon amendment to the Financial Assistance Award, OHA shall adjust monthly allotments as necessary, to reflect changes in the funds shown for MHS 05 Services provided under that line of the Financial Assistance Award; and
(c) OHA may, after 30 calendar days (unless parties agree otherwise) written notice to County, reduce the monthly allotments based on under- under-used allotments funds identified through MOTS and other reports in accordance with the “Reporting Requirements” and “Special Reporting Requirements” sections above or as required in an applicable special conditionscondition;
ii. OHA may, upon written request of County, adjust monthly allotments;
iii. Upon amendment to the Financial Assistance Award, OHA shall adjust monthly allotments as necessary, to reflect changes in the funds awarded for MHS 28 Services provided under that line of the Financial Assistance Award;
iv. (d) OHA is not obligated to provide financial assistance funding for any MHS 28 05 Services that are not properly reported in accordance with the “Reporting Requirements” and “Special Reporting Requirements” sections above or as required in an applicable Specialized Service Requirement by the date 60 calendar days after the earlier of expiration or termination of this Agreement; termination of OHA’s obligation to provide financial assistance for MHS 28 Services; or termination of County’s obligation to include the Program Area in which MHS 28 Services fall within its CMHP; andof
v. County is obligated to report to OHA, by email at xxxxxxxxxxx.xxxxxxxxxxxxx@xxxxx.xx.xx, any TPR payments no later than 30 calendar days following receipt of the Financial Assistance Awardpayment by County or Service Provider.
