You understand and agree that you will be required to notify us of any e-mail address change or name change in writing. We may require verification of your identity and proof of a change in address prior to making any changes in our records. We will only attempt to communicate with you at the most recent address we have on file. If you fail to provide notice of a change in address or name, and we attempt to locate you, we may impose a fee as set forth on the Fee Schedule.
