ELECTION OF ANNUITY FORM. Unless Minnesota Mutual shall be notified in writing to the contrary by the Contract Owner or Participant at least 30 days prior to the Annuity Commencement Date, General Account 14 Accumulation Value will be applied to provide a Fixed Annuity and Separate Account accumulation units will be applied to provide a Variable Annuity.
