Benefit Provisions. The following benefits will be provided to employees based on eligibility requirements in the local collective agreements: (a) Basic Medical Insurance under the British Columbia Medical Plan, subject to Plan provisions. (b) Extended Health Benefits (i) Total lifetime coverage level will be unlimited. (ii) Reimbursement level on claims will be ninety-five percent (95%); where existing reimbursement provisions in a local agreement exceed ninety-five percent (95%), the existing local provision will remain in force. (iii) Hearing Aid benefit claims will be to a maximum of six hundred dollars ($600) every five (5) years. (iv) Medical Travel Referral Benefit shall be in accordance with the provisions set out in Appendix F. (v) Health and welfare benefits coverage will cease on the day that an employee's employment terminates. (vi) Eye vision exams shall be reimbursed to a maximum of seventy- five dollars ($75) every two (2) years. (vii) Effective April 1, 2007 vision care coverage shall be doubled to a maximum benefit of $500 every two (2) years and a minimum benefit of $300 every two (2) years. (c) Group Life and Accidental Death and Dismemberment Insurance Group Life and Accidental Death and Dismemberment benefits each shall be set at three (3) times the employee's annual salary.

Benefit Provisions. A. We will pay covered veterinary services expenses that you incur during the policy term for the diagnosis or treatment of your pet’s condition, up to the limits of this policy. To be eligible for payment, your pet’s condition or procedure to treat this condition must be listed in the VPI® Major Medical Plan Benefit Schedule. B. We will apply your deductible to covered veterinary services expenses that you incur during the policy term. We will pay covered veterinary services expenses that exceed your deductible, up to the limit of one Column A Primary Diagnosis Allowance and any Column B Secondary Diagnosis Allowance that applies to your pet’s condition. These Diagnosis Allowances are the most that we will pay during the policy term for any condition covered by this policy, regardless of the number of incidents or treatments during the policy term. C. Covered veterinary services expenses from each incident are eligible for payment under only one Column A Primary Diagnosis Allowance and any applicable Column B Secondary Diagnosis Allowance. In each incident, we will apply the Column A Primary Diagnosis Allowance of the predominant condition for which your pet received veterinary services. We will not pay both a Column A Primary Diagnosis Allowance and a Column B Secondary Diagnosis Allowance under any Diagnosis Code that applies to the same condition. D. All payments under any Diagnosis Allowance reduce the amount payable under that Diagnosis Allowance for any other covered veterinary services expenses incurred during the policy term. Covered veterinary services expenses that are paid under one Diagnosis Allowance are not payable under any other Diagnosis Allowance. We will only pay veterinary services expenses for diagnostic testing resulting in the diagnosis of a condition that is covered by this policy. E. We will pay for Specialized Diagnostic Tests conducted by your veterinarian, up to the limits of the Specialized Diagnostic Test amounts listed in the VPI® Major Medical Plan Benefit Schedule. We will only pay for tests resulting in the diagnosis of a condition that is covered by this policy. We will not pay more than $1,500 in Specialized Diagnostic Tests per policy term. These Specialized Diagnostic Test allowances apply in addition to any diagnostic testing amounts payable under a Column A Primary Diagnosis Allowance or a Column B Secondary Diagnosis Allowance. F. Benefit Provisions. A. We list your deductible and coinsurance percentage on your Declarations Page or Renewal Certificate. Your de- ductible applies once in each policy term. B. We will pay covered veterinary expenses that you incur during the policy term, subject to your deductible and coinsurance percentage. We will not pay any amount unless your covered veterinary expenses exceed your deductible. If they do, we will: (1) apply your coinsurance percentage to the covered veterinary expenses that exceed your deductible; and (2) pay the resulting amount. C. We will not pay more than $7,500 in each policy term.

Benefit Provisions. 16.2.1 If supervisors are given any options regarding health benefit plans (for example, the choice between Xxxxxx Foundation Health Plan, the District Combined Coverage Medical Plan (PPO) or the District Network Only Plan (EPO) the benefits provided under this section shall be in accordance with the plan or plans under which the retired supervisor was receiving benefits at the time of retirement unless changed during a scheduled open enrollment period. This section shall not be construed as prohibiting any change in benefits or benefit plans as specified in Article 15. 16.2.2 Each retired supervisor and every eligible dependent shall upon obtaining eligibility for Medicare, receive benefits under this section in a manner that augments the benefits that the supervisor or dependent could receive from Medicare even though the retired supervisor or his or her eligible dependents fail to claim rights to Medicare benefits. It shall be the sole responsibility of the retired supervisor and his or her eligible dependents to apply for and satisfy the requirements of Medicare; provided, however, that the District shall reimburse the retired supervisor and his or her eligible dependents for the cost of Standard Medicare Part B premium if adequate proof of payment is submitted to the Office of Human Resources at least once each calendar year and at any time that the amount of the premium is changed by Medicare. 16.2.3 If a retired supervisor or her/his eligible dependents receive benefits under any other health benefits plan, the benefits provided under the other plan shall be primary and the benefits provided under this section shall be reduced to the difference between the benefits provided or paid or payable by the other plan and the maximum benefits provided under this section. 16.2.4 To obtain the benefits provided under this section a supervisor shall file an application for the benefits with the Office of Human Resources and shall complete all necessary enrollment forms prior to the last date of his or her employment with the District.
Benefit Provisions. The benefits provided for in this Article shall not, at the date of signing of this Agreement, fall below the level in existence as described in Appendix C, save and except as they are reduced or altered through circumstances beyond the control of the Employer.
