Electronic Distribution. By checking the “Consent for Electronic Distribution” box, the Participant consents to receive billing, payments, reports, and other distributions electronically. Upon request of the participant OCS will provide paper copies of any document the company sends electronically. The participant may opt out of electronic distributions at any time. Notices of termination or suspension of the project or participation must be delivered in paper form.
