Common use of Electronic Protected Health Information or EPHI Clause in Contracts

Electronic Protected Health Information or EPHI. “Electronic Protected Health Information” or “EPHI” shall have the same meaning as the term “electronic protected health information” in 45 C.F.R. § 160.103, limited to the information created or received by Business Associate from or on behalf of Covered Entity.

Electronic Protected Health Information or EPHI. “The term "Electronic Protected Health Information” " or “"EPHI” " shall have the same meaning as the assigned to such term “electronic protected health information” in 45 C.F.R. CFR § 160.103, limited however, to the information created that Business Associate creates, accesses, or received receives in connection with this BAA or in connection with the Services conducted by Business Associate from or on behalf Associates with respect to patients of Covered Entity.

Electronic Protected Health Information or EPHI. “Electronic Protected Health Information” or “EPHIePHI” shall have the same meaning as the term “electronic protected health information” in 45 C.F.R. § 160.103, limited to the Covered Entity’s information created or received by Business Associate from or on behalf of Covered Entity.

Electronic Protected Health Information or EPHI. “Electronic Protected Health Information” or “EPHI” shall have the same meaning as the term “electronic protected health information” in 45 C.F.R. § 160.103, limited to the information created or received by Business Associate from or on behalf of Covered EntityEntity when Business Associate is a “business associate” as such term is defined in HIPAA.

Electronic Protected Health Information or EPHI. “Electronic Protected Health Information” " or “EPHIePHI” shall have the same meaning as the term “"electronic protected health information” " in 45 C.F.R. § CFR §160.103, limited to the information created which is individually identifiable Protected Health Information that is maintained in or received transmitted by Business Associate from or on behalf of Covered Entityelectronic media.

Electronic Protected Health Information or EPHI. “Electronic Protected Health Information” or “EPHIePHI” shall have the same meaning as the assigned to such term “electronic protected health information” in 45 C.F.R. § 160.103, limited to the information created or received by Business Associate from or on behalf of Covered Entity or Covered Entity’s clients.

Electronic Protected Health Information or EPHI. “Electronic Protected Health Information” or “EPHI” shall have means individually identifiable health information identified as such at or prior to the same meaning as the term “time of disclosure by Covered Entity that is (i) transmitted by electronic protected health informationmedia; or (ii) maintained in electronic media,” in 45 C.F.R. § 160.103, limited to the information created created, received, maintained or received transmitted by Business Associate from Covered Entity from, to or on behalf of Covered EntityBusiness Associate.
