ELIGIBILITY FOR SERVICES. The COUNTY and the WIB shall determine eligibility for receiving services under this agreement.
ELIGIBILITY FOR SERVICES. 1.1 You must be a company, business, partnership or organisation incorporated or registered with the Accounting and Corporate Regulatory Authority of Singapore (ACRA) or such equivalent authority in your country of incorporation or registration. We may decline acceptance of your subscription at our discretion. We will be deemed to have accepted your subscription if we have made available the Services to you regardless of any written acceptance, confirmation or agreement from us to you or your actual use of the Services. 1.2 The provision of all our Services is subject to us obtaining the necessary approvals, permits, licences from the respective regulatory Authorities. The start date for our services is an estimated date and may be subject to change due to various factors including, but not restricted to, weather conditions, safety issues, unforeseen factors, soil condition, additional works, etc. Where the service start date changes, we will inform you in writing or e-mail as soon as practical once all the information is known and at hand to ascertain the revised start date.
ELIGIBILITY FOR SERVICES. 1. The parties understand and agree that the County of Financial Responsibility shall have the responsibility of determining the eligibility of the person to receive Purchased Services in accordance with the plan of eligibility criteria established by applicable Minnesota Rules and federally approved state waiver plan requirements, and under MN Statute 256B.0913. 2. When the County of Financial Responsibility has determined the person is no longer eligible to receive services or that services are no longer needed or appropriate, the County of Financial Responsibility shall notify the person or the person’s legal representative in writing of the proposed termination, denial, or reduction of services within ten (10) business days prior to the County of Financial Responsibility’s proposed date of action. The County of Financial Responsibility shall also notify the Contractor within ten (10) business days of the determination. 3. The County of Financial Responsibility shall also provide information regarding the person’s right to appeal the proposed County of Financial Responsibility’s action as provided under Minnesota Statutes, section 256.045.
ELIGIBILITY FOR SERVICES. The parties understand and agree that the eligibility of a family to receive Peer Support Specialists, Recovery Coaches, or Peer Mentor Services shall be determined by the Department. When the Department has determined that the family is no longer eligible to receive Peer Support Specialists, Recovery Coaches, or Peer Mentor Services from the Contractor, the Department shall notify the Contractor in writing within 10 working days of this determination. The Contractor will be paid for all Peer Support Specialists, Recovery Coaches, or Peer Mentor Services rendered prior to receipt of said written notice.

ELIGIBILITY FOR SERVICES. Children and youth ages 3-17 experiencing a mental health crisis including children currently or previously DFPS involved, at risk of parental relinquishment or removal, multiple or extended stay inpatient psychiatric hospitalizations, multiple placement disruptions, high behavioral health acuity, Juvenile Justice Involvement, and multiple referrals for behavioral and/or mental health services within the school district. Individuals at risk for low-level controlled substance-related offenses and misdemeanor crimes that are identified as appropriate for co-response will be included as eligible for YCRT Program Services. The Program shall prioritize focus on crisis intervention and participants with histories of repeated involvement with law enforcement.
