Eligibility Screening. GRANTEE shall conduct eligibility screening for each individual seeking victim-related civil legal services. XXXXXXX agrees to use the intake screening form supplied by the OAG. A copy of all intake screening forms shall be maintained by GRANTEE in a central location and shall be made available to the OAG or any party the OAG determines should have access to these documents at any reasonable time.

Eligibility Screening. INITIAL COMPLIANCE SCREENING HHSC will perform an initial screening of all applications received. Unsigned applications and applications that do not include all required forms and sections are subject to rejection without further evaluation. If the application passes the initial screening, the Applicant will be contacted by DDS staff for further instructions or actions.

All Rent Relief Program applications must be completed by the applicant and submitted to Silicon Valley Independent Living Center.
