Eligible Population. 5.1 Program eligibility is determined by applicable law set forth in Program rules and the requirements established in the Program Policy Manual. 5.2 The unduplicated number of Clients for PHC services is 430. This represents the Grantee’s projected number of unduplicated Clients to be served during the Contract period. If during the Contract period it is foreseen that the Grantee might be unable to serve the contracted number of children, HHSC may reduce the Grantee’s grant award amount.

Eligible Population. The CMHG program supports a range of clinical mental health and non-clinical supportive services for persons with unmet mental health needs. The eligible population to be served consists of Texas residents (i.e., children or adults) who have a clinical diagnosis of a mental health condition and/or co-occurring disorders, including, but not limited to, intellectual or developmental disability, substance use disorder, and complex medical conditions. B. Implementation 1. -approved the standards further described in this Statement of Work.
Eligible Population. 6.1 Program eligibility is determined by applicable law set forth in Program rules and the requirements established in the Program Policy Manual. 6.2 The contracted number of children is for Autism services is 27. This represents the Grantee’s projected number of unduplicated children to be served during the Contract period. If during the Contract period it is foreseen that the Grantee might be unable to serve the contracted number of children, HHSC may reduce the Grantee’s grant award amount in order to encumber funds under another Autism services contract so that additional children may be served, or to expend the funds on other appropriate purposes.
Eligible Population. A. PATH funds are for the purpose of providing services to adult individuals who: 1. Have serious mental illnesses and may have co-occurring substance use disorders; and 2. Are homeless or at imminent risk of becoming homeless. B. Individuals may be served in the outreach setting, and when appropriate, enrolled for the sole purpose of engaging the human service agencies, mental health services, or education system to provide services to the individual.
Eligible Population. Grantee shall provide services to individuals who are Texas residents and meet one of the following Client types: 1. A pregnant woman; 2. The biological father of an unborn child; 3. The biological parent of a child who is 36 months of age or younger; 4. An adoptive parent of a child who is 36 months of age or younger; 5. An approved adoptive parent of an unborn child; 6. A former Client who has experienced the loss of a child; 7. A parent or legal guardian of a pregnant minor who is a program Client; 8. Parent, legal guardian, or adult caregiver of a child who is 36 months of age or younger; or 9. A parent who experienced a miscarriage or loss of a child not more than ninety (90) calendar days before the parent begins participation in the services offered through the program.
Eligible Population. Eligibility for family violence services is determined solely on a survivor's self-reported experience with family violence, domestic violence, and/or dating violence. Those receiving enhanced services paid for with Federal Funds must have a household income less than 200 percent of the federal poverty level. Grantee must provide services and/or education to the population in accordance with Attachment L.

Eligible Population. The eligible population to be served under this Grant Agreement is: 1. Individuals who are 60 years or older; 2. Individuals with intellectual disabilities, developmental disabilities, physical, and/or chronic diseases or conditions; 3. Family caregivers; 4. Veterans; 5. Families with children with disabilities; and 6. Other individuals planning for future LTSS needs.
Eligible Population. Women with dependent children (including women whose children are in custody of the state, if the children are allowed to enter treatment with the woman) and pregnant women in the third trimester of their pregnancy. Women shall be Texas residents who meet financial criteria for System Agency -funded substance use disorder services and meet The Diagnostic and Statistical Manual of Mental Disorders criteria for a moderate or severe substance use disorder (substance dependence).
Eligible Population. Any individual who uses substances. This population may include, but is not limited to, adults and youth, populations who are marginalized or stigmatized, experiencing housing instability or homelessness, inject substances, live with or at risk of Hepatitis C Virus (HCV) or Human Immunodeficiency Virus (HIV), in need of medical and or mental health services, experience greater barriers to entering treatment or recovery services, and those seeking to enhance their recovery capital and maintain their recovery from substance use disorders.
