Common use of Enrollment, Cost and Utilization Reports Clause in Contracts

Enrollment, Cost and Utilization Reports. (A) The Contract shall submit Enrollment, Cost and utilization reports in an electronic format designated by the Department. The reports shall be in Excel, and the Contractor shall utilize the Excel template provided by the Department. The Contractor is not allowed to customize or change the format of this report. The Department may amend the report at its discretion.
