- Evaluation and Scoring of Proposals. The Proposal Review Team will evaluate based on Xxxxxx’s proposal submission. Proposal scoring will be weighted as described in this section. - Evaluation and Scoring of Proposals. (The rest of this page is intentionally left blank) PROPOSAL FORM 3: DIVERSITY VENDOR CERTIFICATION PARTICIPATION Diversity Vendor Certification Participation - It is the policy of some Members participating in Equalis Group to involve minority and women business enterprises (M/WBE), small and/or disadvantaged business enterprises, disable veterans business enterprises, historically utilized businesses (HUB) and other diversity recognized businesses in the purchase of goods and services. Respondents shall indicate below whether or not they hold certification in any of the classified areas and include proof of such certification with their response.
