Evidence Required for Extended Illness Leave. Upon return to work, an employee shall complete an application for Extended Illness Leave form to justify the use of Extended Illness Leave. The Employer may, when an employee utilizes any leave, including unpaid leave for medical appointments or when the absence is five (5) consecutive work days or more, require the employee to furnish a certificate from a physician, dentist or other medical practitioner. Falsification of either a written, signed statement or practitioner's certificate shall be grounds for disciplinary action including dismissal.
