EXAMINATION REPORT. 1. Each employee may request an examination by his/her personal physician every three (3) years. The results of such examination shall be reported upon a form furnished by the Board. 2. The cost of each initial and three (3) year physical examination will be paid by the Board, except that the employee shall submit the xxxx to his/her insurance company and the Board will pay the balance not covered by insurance.
