Final Appeal Process. Any Provider Appeal that is not resolved to Provider’s satisfaction during the Initial Appeal Process must be submitted to the Final Appeal Process. All Provider Appeals must be submitted to the Final Appeal Process by Provider, in writing, within ninety (90) days of Blue Shield’s Initial Provider Appeal determination, to the address for such Provider Appeals provided on the provider portal of Blue Shield’s website at xxx.xxxxxxxxxxxx.xxx.
