FORMAT OF PERFORMANCE EVALUATION. The performance evaluation shall be made on a standard form approved by the City Manager, which provides for the signature of the employee as evidence that he has been informed of his rating and has had an opportunity to discuss it with the supervisor. The standard form provides a uniform and consistent method of evaluating employee performance related to the assigned duties and responsibilities of the employee. A brief description of each performance area is included in the standard form. These definitions are not all inclusive, but deliberately general in nature. Supervisors are encouraged to use extra paper when additional space for comments, is needed, etc.
