Health Care Agent. By executing a form known as the Health Care Proxy, you may authorize an individual to make health care decisions for you in the event that you are no longer able to make those decisions for yourself. Facility encourages all residents to carefully consider and to execute a Health Care Proxy. If you have already designated someone to be your Health Care Agent, you are required to provide Facility with a copy of your Health Care Proxy form. The Social Worker can assist you in completing a Health Care Proxy form if you have not already designated someone to act as your Health Care Agent.
