Common use of Health Care Professional Clause in Contracts

Health Care Professional. A physician or any of the following: podiatrist, optometrist, chiropractor, psychologist, dentist, Physician Assistant, physical or occupational therapist, therapist assistant, speech-language pathologist, audiologist, Registered or practical Nurse (including nurse practitioner, clinical nurse specialist, certified Registered Nurse anesthetist and certified nurse midwife), a licensed certified social worker, registered respiratory therapist and certified respiratory therapy technician.

Health Care Professional. A physician or any of the following: podiatrist, optometrist, chiropractor, psychologist, dentist, Physician Assistantphysician assistant, physical or occupational therapist, therapist assistant, speech-language pathologist, audiologist, Registered registered or practical Nurse nurse (including nurse practitioner, clinical nurse specialist, certified Registered Nurse registered nurse anesthetist and certified nurse midwife), a licensed certified clinical social worker, registered respiratory therapist and certified respiratory therapy technician.

Health Care Professional. A physician or any other provider of the followinghealth care services under this Demonstration, including but not limited to: a podiatrist, optometrist, chiropractor, psychologist, dentist, Physician Assistantphysician assistant, physical or occupational therapist, therapist assistant, speech-language pathologist, audiologist, Registered registered or practical Nurse nurse (including nurse practitioner, clinical nurse specialist, certified Registered Nurse anesthetist registered nurse anesthetist, and certified nurse midwife), a licensed certified social worker, registered respiratory therapist therapist, and certified respiratory therapy technicianassistant.

Health Care Professional. A An individual with current and appropriate licensure, certification, or accreditation in a medical or behavioral health profession, including without limitation, medical doctors (including psychiatrists), dentists, osteopathic physicians, psychologists, registered nurses, nurse practitioners, licensed practical nurses, certified medical assistants, licensed physician or any of the following: podiatristassistants, optometristmental health professionals, chiropractorchemical dependency counselors, psychologistclinical laboratory professionals, dentistallied health care professionals, Physician Assistantpharmacists, social workers, physical or therapists, occupational therapisttherapists, therapist assistant, speech-language pathologist, audiologist, Registered or practical Nurse (including nurse practitioner, clinical nurse specialist, certified Registered Nurse anesthetist and certified nurse midwife), a licensed certified social worker, registered respiratory therapist and certified respiratory therapy technicianothers to provide Health Care Services.
