Extended Health Care Plan. ‌ (a) The Employer shall pay the monthly premiums for extended health care coverage for employees and their families under the Pacific Blue Cross plan. The maximum lifetime amount payable per eligible employee or eligible dependant shall be unlimited. (b) There shall be coverage for eye glasses and hearing aids. The allowance for vision care will be $350.00 every twenty-four (24) months per eligible employee or eligible dependant; the allowance for hearing aids will be $600.00 every forty-eight (48) months per eligible employee or eligible dependant. (c) During the term of this Agreement Pacific Blue Cross will be the carrier of the extended health care plan. (d) The parties agree to Pharmacare Tie-In effective April 1, 2013.

Please refer to the Employee Benefit Book for specific details and conditions that may apply to the above-noted benefit(s). In the event of any conflict between the descriptions above and the Benefit Booklet, the terms of the Benefit Booklet and insurance will govern.

Extended Health Care Plan. This plan is an insured plan providing for limited payments for additional medical benefits not covered by the Ontario Health Insurance Plan. The total premium of this plan is paid by the University of Waterloo. An employee and the University share the cost of eligible benefits under this Plan on a 20% / 80% basis respectively. Maximum dollar benefits may be applicable and there are maximum annual costs (caps) to the employee’s out of pocket expenses for eligible benefits under the Plan. Maximum dollar benefits and caps are subject to adjustment annually by the Pension and Benefits Committee.
Extended Health Care Plan. (a) The Employer shall pay the monthly premium for regular employees entitled to coverage under a mutually acceptable Extended Health Care Plan. (b) The plan is to make provision for transportation for an employee or an eligible dependant to medical services not available in the employee's home community. (c) Where upon the written advice of a medical practitioner a person eligible under Clause 24.2(b) requires a travelling companion, transportation costs for the companion shall also be paid. (d) The plan shall also provide for expenses of up to fifty dollars ($50) per day up to a maximum of seven (7) days per referral in cases where transportation has been provided under Clauses 24.2(b) and (c) above. There shall be no doubling up of the daily maximum for the patient and the travelling companion.
