Common use of Health Home Services Clause in Contracts

Health Home Services. The MCO must provide Health Home Services. The MCOs must include a designated Provider to serve as the health home. The designated provider must meet the qualifications as established by the U.S. Secretary of Health and Human Services. The designated provider may be a provider operating with a team of health professionals, or a health team selected by the enrollee. The Health Home Services must be part of a person-based approach and holistically address the needs of persons with multiple chronic conditions or a single serious and persistent mental or health condition. Health Home Services must include: 1. patient self-management education; 2. provider education; 3. evidence-based models and minimum standards of care; 4. standardized protocols and participation criteria; 5. provider-directed or provider-supervised care; 6. a mechanism to incentivize providers for provision of timely and quality care;
