Common use of Health Records Clause in Contracts

Health Records. Contractors must maintain current and confidential (A-13) health files on all enrolled children that include:  Medical and dental history.  Immunization records.  Allergy information.  Food preferences and restrictions.  Health screening results (D-7).  Dental screening results (D-8).  Medical examination records (D-9).  Accident reports.  Documentation of health-related family contacts. Contractors must maintain a tracking system to ensure these records are kept up-to-date. Contractors must maintain documentation that they:  Notify parents that staff are mandated reporters of suspected child abuse and neglect in accordance with RCW 26.44 (D-3).  Notify parents, employees, and any other interested parties 48 hours in advance of the application of pesticides in accordance with RCW 17.21. Notification is not required if children will be out of the facility for two consecutive days after application.  Notify parents that children have been exposed to infectious disease and parasites in accordance with WAC 170-295-3010.
