HMO Dental Coverage Sample Clauses

HMO Dental Coverage. For the term of this Agreement, the City shall contribute thirty-one dollars and ninety-five cents ($31.95) per month towards a HMO dental benefit plan for employee and his/her qualified dependent(s). Any amount necessary to cover the monthly premium in excess of the City’s contribution is the responsibility of the employee.
