Hospital System Annual Report. Hospital systems must submit an annual report, based on the timeline approved in the reporting protocol component of the master DSTI plan. The reports must at a minimum: Be submitted using a standardized reporting form approved by the state and CMS; Provide information included in the semi-annual reports, including data on the progress made for all milestones; and Provide a narrative description of the progress made, lessons learned, challenges faced and other pertinent findings.
