Information Required on Application. Applications for catastrophic illness or injury sick leave donation must be submitted to the Chief People Officer, or designee. Applications will include, but not be limited to the following information:‌ a. the nature of the claimed catastrophic illness or injury; b. for whom the application is being submitted; c. documentation of physician(s) diagnosis and prognosis of the catastrophic illness or injury; d. projected date of return to duty; e. explanation of previous leave usage; and f. any other pertinent information the applicant can submit to the committee for its consideration.
