Application Requirements. A written application for sabbatical for a subsequent academic year, which includes the reasons for requesting the leave and a detailed description of how the proposed sabbatical plan meets the criteria herein, shall be submitted to the professional development committee (PDC) by November 1. The appropriate administrator and Vice President of Instruction will also be included upon initial review of the application and will provide written recommendation to the President.

Application Requirements. Each non-public school desiring services or assistance under this subsection shall submit an application to the State educational agency at such time, in such manner, and accompanied by such information as the State educational agency may reasonably require to ensure expedited and timely provision of services or assistance to the non-public school, which shall include-- (i) the number and percentage of students from low-income families enrolled by such non-public school in the 2019–2020 school year; (ii) a description of the emergency services authorized under paragraph (4) that such non-public school requests to be provided by the State educational agency; and (iii) whether the non-public school re questing services or assistance under this subsection received a loan guaranteed under paragraph (36) of section 7(a) of the Small Business Act (15 U.S.C. 636(a)) that was made before the date of enactment of this Act and the amount of any such loan received.
Application Requirements. In order for Resident to perform professional services as required by Baptist in this Agreement, Resident acknowledges and agrees that certain application requirements should be timely and accurately met by Resident prior to the start date of the initial term of this Agreement. In order for Baptist to provide professional liability insurance and for Baptist to begin paying Resident for resident services, Resident must complete Baptist's minimum application requirements, as separately provided by Baptist, at least thirty (30) days prior to the start date of this Agreement. Resident further acknowledges that Baptist may require additional information beyond its minimum requirements, and Resident agrees to timely and accurately provide such information by the date(s) requested by Baptist. In the event Resident cannot meet these application requirements by the date listed above, Resident shall notify Baptist in writing, in accordance with Section 10 of this Agreement, of the specific application items to be outstanding, any reasons for delay, and any problems with the application process. Resident hereby affirms that any information submitted in Baptist's application process shall be true and complete to the best of Resident's knowledge, and Resident shall have an ongoing obligation to inform Baptist immediately upon becoming aware of any material change in Resident's application information.

Application Requirements. The Participating Contractor shall provide NYSERDA the information below when submitting the Participation Agreement Application Signature Form, as requested by NYSERDA, or when there are changes or updates to the information previously provided. (a) Completed Participation Agreement Application Signature Form: The Applicant must read and submit the completed Agreement Application Signature Form to NYSERDA indicating agreement with its terms. The Agreement Application Signature Form must be submitted by an individual with the full power and authority to enter into an Agreement on behalf of the Company. (b) Detail of company and staff experience in the energy efficiency sector. This is a requirement for all new applicants and for returning applicants at NYSERDA’s request. (i) A minimum of three completed references detailing relevant energy efficiency projects performed within the past six months. The reference must contain a detailed description of the work performed and customer contact information. (ii) Employee roster of both certified and non-certified employees providing work experience, previous firms worked at and training. The contractor must provide sufficient information for Program staff to make a determination on the qualifications of a contractor to perform work through the Program. It is NYSERDA’s sole discretion to request additional information as necessary for determining the eligibility of an applicant in meeting the requirements of Section (iii) Home Performance Contractors should provide documentation for a minimum of 6 months experience for each type of specialty measure work they are looking to perform through the Program. If for example a contractor is looking to perform Oil Heat Work through the Program, they should document how many installs they have performed for the past 6 months, type of equipment installed, and reference of the standards/procedures used (BPI, manufacturers, XXXX) during a typical installation. (c) Certificate of Insurance (d) Dba form (if applicable)
