Less than Five-Day Notice. For a period of time off of less than five 1 consecutive days, the nurse shall make the request to the supervisor at least two 2 weeks prior to the date requested. In the event two or more nurses in a 3 department request the same day(s) off, the Hospital shall seek to accommodate 4 the requests; but, in the event scheduling will not permit, date of request shall be 5 determinative.
