Licensure Status. Resident shall apply for an unrestricted Wisconsin license to practice medicine as soon as Resident becomes eligible. If Resident is unable to obtain a valid Wisconsin medical license within a reasonable period of time after submitting an application, or, in the event that Resident secures a Wisconsin medical license and it is subsequently restricted, revoked or suspended, this Agreement shall automatically terminate. In the event that Resident is not yet eligible to apply for a Wisconsin license to practice medicine but is eligible to apply for a Wisconsin Resident Educational License (“REL”) Resident shall apply for and secure a REL and shall maintain or renew the REL until he/she is eligible for and secures an unrestricted medical license.
Licensure Status. In the event the Board appoints an Athletic Trainer who is not certificated to teach, but who holds the appropriate license issued by the State Board of Medical Examiners, then the following provisions will apply:
A. Salary shall be governed by a separate non-certificated Athletic Trainers' Salary Guide, which is attached hereto and made a part hereof. The Trainer shall possess a Bachelor's degree in order to qualify for guide placement.
B. Fringe benefits and workday shall be the same as provided in 1.B. and 1.C. above.
C. Work year shall be ten and one half (10½) months, which includes two hundred and twenty (220) workdays per year.
Licensure Status. At all relevant and material times, Licensee held the following license to practice as a pharmacist in the Commonwealth of Pennsylvania: license no. [number here], which was originally issued on [date here], and which is currently set to expire on [date here].
Licensure Status. At all times relevant to this case, Respondent held the active Connecticut CPA firm permit #CPAP.0005388 and is subject to the Board’s jurisdiction pursuant to C.G.S. §20-280b.
Licensure Status. At all relevant and material times, Respondent held the following license to practice as a medical physician and surgeon in the Commonwealth of Pennsylvania: license no. MD066378L, which was issued on January 5, 1999 and is active through December 31, 2022.
