Long Term Disability Coverage – Basic. 1. The District will continue to maintain Basic Long Term Disability (LTD) coverage with a maximum monthly benefit of sixty six and two thirds percent (66 2/3%) of the base monthly salary subject to the following maximum benefit amount: After 26th week $3,335 per mo. $5,000 per mo. 2. Employee Paid Long-Term Disability Coverage - Voluntary Employees may purchase additional Long Term Disability Insurance to insure payment of up to sixty-six and two thirds percent (66-2/3%) of the base monthly salary subject to the above-listed maximum benefit amount. Premiums for this additional coverage shall be fully paid by the employee. Eligibility of employees to purchase additional Short Term and Long-Term Disability coverage shall be subject to medical certification of insurability at the employee’s own expense.
