Disability Coverage. In the event a State employee goes on an extended medical disability, or is receiving Workers’ Compensation benefits, the Employer-policyholder shall continue at no cost to the employee the coverage of the group life insurance for such employee for the period of such extended leave, but not beyond two (2) years.
Disability Coverage. In the event a bargaining unit employee is receiving disability leave or Workers’ Compensation benefits, the Employer shall continue payments to the Trust pursuant to Section 54.01 for the period of such extended leave, but not beyond two (2) years.
Disability Coverage. Teachers who have been approved for benefits under the School District’s long term disability insurance plan or the Workers’ Compensation Act and who previously enrolled in the School District’s health and hospitalization, dental and life insurance benefits will have these benefits maintained for a period of one (1) year from the date of qualification for the disability benefit. Thereafter, these benefits will be available to the teacher at their expense subject to the provisions of the insurance carrier plan description.
Disability Coverage. Long-term and short-term disability coverage equal to 66-2/3% of Base Salary and Average Annual Bonus. For purposes of this Agreement, "Average Annual Bonus" shall mean the average of the three (3) most recent annual Cash Bonuses paid to the Employee immediately preceding the determination date.
Disability Coverage. The employer agrees to pay the premium for long-term disability benefits for all regular employees who have completed their probationary period and who work more than 20 hours per week. The LTD plan shall provide for a ninety (90) calendar day waiting period and shall pay 66 2/3% of salary to a maximum of $1,000 per month. This LTD insurance shall be subject to the terms and conditions of the carrier and does not waive the at-work requirement for an employee to receive the increase in this benefit. See Appendix B.
Disability Coverage. The District will no longer provide the Peace Officers’ Research Association of California (PORAC) sponsored Premier Plus ― Long Term Disability Plan (hereinafter referred to as PORAC) for all BPMA members.
Disability Coverage. The District agrees to continue payments for all benefit programs provided in Sections 12.1 and 12.2 of this Article during the absence of any employee in the bargaining unit who is on paid sick or disability leave for the extent of sick leave. Additional coverages and benefits under the California Family Rights Act and the Family Medical Leave Act are available to all bargaining unit employees (please see Article XV, Leaves).
Disability Coverage. Short- and long-term disability coverage that is reasonably comparable to the coverage provided to the Executive on his Termination Date and which can be purchased on the open market shall be for a period equal to the lesser of twenty-four (24) months following the Executive's Termination Date or until the Executive is eligible to receive comparable benefits through another employer;
Disability Coverage. 16.1 Full-time employees who have completed ninety (90) days of continuous employment shall be eligible for disability benefits beginning on the first day in the event of an accident not subject to Workplace Safety & Insurance Board and beginning on the third day in the event of sickness. Benefits are calculated on the basis of seventy-five per cent (75%) of an employee's basic rate of pay with a minimum hourly rate of $12.50 and are payable for up to a maximum of seventeen (17) weeks per year non- cumulative. In the event that an employee's sickness results in hospitalization during the period of such sickness, benefits shall commence from the first day of sickness.
16.2 The Company reserves the right to change insurance companies or to self-insure, as the case may be, on the understanding that the benefits forthcoming to the employees are not affected by such change.
16.3 An employee injured on the job and required to go to a physician’s office or hospital will be provided free transportation by the Company, and where necessary, back to the Company’s premises or his or her home.
16.4 If an employee is injured on the job during regular working hours and as a result of being properly authorized to leave work, does not perform all regularly scheduled working hours of the shift on such day, the Company will make up his pay for his regularly scheduled hours so lost, at his base hourly rate of pay exclusive of premiums.
Disability Coverage. The City shall provide a long-term disability plan that provides for two-thirds (2/3rds) of the employee's earnings, should the employee become disabled and not able to perform work.
