Common use of Mandatory Reporting Clause in Contracts

Mandatory Reporting. Contractor shall report to the Department in the manner set forth in Subsection 22(m) any use or disclosure or suspected use or disclosure of Confidential Information not provided for by the Contract, of which it becomes aware, including breaches or suspected breaches of unsecured Protected Health Information as required at 45 CFR 164.410.

Mandatory Reporting. Contractor shall report to the Department in the manner set forth in Section 24.0 , Subsection 22(m(m) Contractor Reporting of Breach or Suspected Breach or Disclosure to the Department any use or disclosure or suspected use or disclosure of Confidential Information not provided for by the Contract, of which it becomes aware, including breaches or suspected breaches of unsecured Protected Health Information as required at 45 CFR 164.410.
