Mayo Executive Physical Program. If you are entitled to receive Change in Control Benefits, the Company will, for a period of 12 months following your termination of employment, continue to allow you to participate in the Mayo Executive Physical Program and cover all regularly authorized expenses associated therewith, including, without limitation, travel, meals, lodging and fees. In order to be reimbursed, all such expenses must be submitted promptly and no reimbursements will be made following the December 31 of the second calendar year following the calendar year in which your Separation from Service occurs.
