Program Participants and their counsel further shall provide Organon with any information necessary for Organon to meet any mandatory reporting requirements specific to each states’ Medicaid or other governmental agency reporting and reimbursement laws and regulations. Any Program Participant who was or is a Medicaid beneficiary will execute and provide the information and forms requested in the Medicare/Medicaid Addendum and Release contained in Appendix I.
