Common use of Medication Control Clause in Contracts

Medication Control. All medications shall be locked in a central secured location accessible only to authorized staff; exceptions shall be documented by a licensed medical professional stating that the medication does not need to be in a central secure location. Self-administration of all medications and treatments shall be supervised, and assistance provided as needed. A licensed nurse or medical professional shall monitor medication records. Medication will be prescribed by a psychiatrist or other licensed medical professional. Using the recommendations from the medical screening and assessments, the facility licensed nurse and Program Director shall develop a related plan for the client. The plan may include: o training staff and clients on the medications’ doses and administration o where authorized by a licensed medical professional, an individual medication- management plan may be developed for a client o for all cases, the plan will include documentation that states times and doses of medications self-administered by clients. All unused or discontinued prescribed medications shall be destroyed or returned to a pharmacy, and this will be noted in the specific client’s centrally stored medication record. • Medication Monitoring – Medication monitoring will be conducted as needed, and at least every 60 days, by the client’s treating physician/psychiatrist. The criteria reviewed will include: o Appropriateness of medications ordered based on documented and presenting symptoms and diagnosis o Appropriateness of dosing and titration of individual medications o Appropriateness of monitoring of individual medications o Cost-effectiveness of prescribing patterns The physician/psychiatrist will report changes, based on his or her review, to the Program Director and interdisciplinary team, as well as in writing; the review will be documented immediately in the client’s chart. Results and changes will also be reported to the referring county as needed or requested. In addition to monthly medication monitoring and review, all clients will have access to their treating physician/psychiatrist as needed.
