Grantee/Contractor shall notify in writing its assigned System Agency contract manager of any insolvency, incapacity, or outstanding unpaid obligations of Grantee/Contractor owed to the Internal Revenue Service or the State of Texas, or any agency or political subdivision of the State of Texas within five business days of the date of Grantee/Contractor’s becoming aware of such. Attachment F PROGRAM REQUIREMENTS (STATEMENT OF WORK) A. Grantee shall submit the following reports in their original format: Report Frequency Due Date Monthly Progress Report that includes: A. Clients served; B. Monthly Progress Summary; and C. Monthly Outreach activities The fifth (5) day of the following month clients were served. If the due date falls on the weekend, the report will be due on the Monday after.
